Studies describing avoidable hospital transfers for nursing home (NH) residents sug-gest rates vary between 30% and 70%, often citing poor communication, limited resources, patient/family preference, and adverse events as underlying causes.1-3Adverse events, defined as harm resulting from the delivery of care, are particularly concerning because by definition they are often preventable events.4The Office of the Inspector General identified harm related to adverse events affected one-third of Medicare skilled nursing facility residents, including events resulting in $2.8 billion in hospitalization costs.
